- C/O Form EAV: The form with all information correctly filled out.
- Supplementary Declaration for C/O (if any): Based on Appendix V of Circular No. 21/2016/TT-BCT dated September 20, 2016.
- Printed Export Customs Declaration: If the exported goods do not require a customs declaration, this document is not needed.
- Copy of Commercial Invoice: Stamped as a true copy by the trader.
- Copy of Bill of Lading or Transport Document: Stamped as a true copy by the trader. Mandatory if the exported goods use a delivery method without a bill of lading or transport document as per legal regulations.
For trader registration documents, you need:
- Signature and Seal Registration: Signature sample of the authorized person signing the C/O application and the trader’s seal.
- Copy of Business Registration Certificate: Stamped as a true copy.
- List of the trader’s production facilities.
- Application for changing the place of C/O issuance.
Note: If you have not registered the trader dossier before exporting goods, you need to register and submit the trader registration dossier as required by law.
3. Main contents of CO form EAV
C/O Form EAV includes various information fields, covering details about the exporter, importer, transport information, reference number, item number, type and number of packages, goods description, origin criteria, quantity, invoice date and number, date of issuance of the certificate of origin, and origin of the goods. - Box 1: Information about the exporter: trading name, address, country.
- Box 2: Information about the importer and consignee: trading name, address, country.
- Box 3: Transport information: departure date, means of transport, place of unloading.
- Box 4: Unique reference number, name of the country issuing and submitting the certificate of origin.
- Box 5: Name of certifier or replacement, number, and date.
- Box 6: Item number.
- Box 7: Number and type of packages.
- Box 8: Goods description, HS code of the importing country, product model, brand (if any).
- Box 9: Origin criteria for goods.
- Box 10: Quantity, weight (kg), actual weight not exceeding 5% of the weight stated on the C/O.
- Box 11: Invoice date and number submitted to the C/O issuing authority.
- Box 12: Date of C/O issuance, place of issuance, signature, and seal of the authorized person.
- Box 13: Origin of goods (Eurasian Economic Union member countries or Vietnam), declaration date, place of declaration, seal, and signature of the declarant.
